Postgres RLS, not app checks

Row-level security is enforced at the database. A bug in the app can't leak another practice's records — the query is refused.

PHI sanitizer on every AI call

Names, DOBs, phones, and identifiers are stripped before any prompt leaves the machine. The model sees clinical signal, never identity.

A BAA with every party

Anthropic, Supabase, LiveKit, and anyone added later must sign a Business Associate Agreement before production PHI flows through them.
