Read the draft
The scribe has already written it from the visit. You are reading, not typing.
Step 2
Correct what is wrong
Your edits are yours, and they are recorded as yours.
Step 3
Check the code against the note
The CPT and ICD were drawn from the note and the plan and checked against the payer’s rules. The note still has to support them, and that judgement is yours.
Step 4
Sign
The signature is what turns the note into the 837 that goes out the same night. In the September close, notes were signed for 12 of 12 billed visits.
Step 5
Know what an unsigned note costs
Notes signed for billed visits is one of the checks in the monthly close, so an unsigned note holds the close until somebody signs it.
