A chart is a sequence of visits, each with a note, a code and a claim, and whatever the patient did in between. This lesson reads one end to end.

Step 1
Move down the visits
Each visit carries its note, the code it was billed under, and the claim that came out of it.
Step 2
Open a note
SOAP or ADIME in the practice’s own template, with the PES statement in it and the care plan changes underneath.
Step 3
Follow a line back
Every number on the record has a receipt behind it: the action that made it, who took the action, and when. Open one.
Step 4
Read the trend, not only the visit
Weight, A1C and intake are trended across visits. A flag on a trend is a clinical fact, and it is what triage will act on tomorrow.
