Read the reason code
It is on the held claim, against the line it applies to.

Find the evidence
If the benefit was verified before the visit, the reference number, the recording and the transcript are already on that visit.
Step 3
Answer in the payer’s own words
An appeal that quotes the representative who confirmed the benefit, with the date and the reference number, is a different document from one that argues.
Step 4
Keep the thread on the claim
The appeal, the reply and whatever it changed stay on the claim they belong to, so the next person can read the whole story.
