Step 1
Coding
CPT and ICD are drawn from the note and the plan and checked against the payer’s rules before the claim leaves the building.
Step 2
The 837
The claim goes out through Office Ally, Availity, or the payer directly, with its status visible on the record it came from.
Step 3
Tracking
Accepted, rejected, paid. The status sits on the visit, not in a separate system somebody has to remember to open.
Step 4
The 835
The remittance posts itself to the ledger and matches against the claim. In the pilot, 98% of claims were paid on first pass.
Step 5
What is held, and why
A short-pay or a denial is held with its reason, and the verification taken before the visit is sitting beside it.
