Charting and the care plan

Triage, and what it surfaces

the plate for the lesson Triage, and what it surfaces

Every morning the practice opens on the people who need attention, ranked, with the reason in one line.

Where it happens

The day’s screen, in auro EHR. This lesson is part of the Charting and the care plan track.

What this lesson covers

  1. Step 1

    The four tiers

Urgent, active, review, ok. The tier is set by the logs, the weight trend, the missed check-ins and the flags a dietitian raised.

  2. Step 2

Read the reason, not the tier

Each row says in one line why it is where it is. The tier is the sort order; the reason is the clinical content.

  3. Step 3

    Engagement dropout is clinical

No food logged in fourteen days surfaces here, before the next visit is booked rather than after it is missed.

  4. Step 4

    Act from the row

Send a check-in, add a note, escalate, or hand it to the coordinator without leaving the row you are on.

After this lesson

You can work the triage list top to bottom and leave every row either done or handed on.

The Academy keeps a record of what each person has completed. It is a record, not a credential: auro accredits no one, and no lesson carries CE or CEU credit.
