Step 1
Read the drafted problem
It comes from the visit and from what the patient logged, not from a list of common ones.
Step 2
Test the etiology
This is the clause that has to be defensible. The note has to say why the problem exists, not only that it does.
Step 3
Test the signs and symptoms
Each should point at something on the record: a log, a measurement, a lab, a sentence in the transcript.
Step 4
Rewrite what is not yours
The statement carries your name the moment the note is signed, so it has to read the way you would have written it.

Watch it reach the care plan
Targets, interventions and follow-ups change with the statement, and they follow the patient home in the app.
